Accessibility statement
This accessibility statement applies to the NHS England web application:
Calculating Quality Reporting Service (CQRS) National web application: cqrs.nhs.uk
The application is managed by the CSU Collaborative which represents the four NHS Commissioning Support Units (CSUs) on behalf of NHS England and is hosted by Arden and Greater East Midlands (AGEM).
NHS England is committed to ensuring the CQRS National web application is accessible to as many people as possible. This means you should be able to:
- change colours, contrast levels and fonts using browser or device settings
- zoom in up to 400% without the text spilling off the screen
- navigate most of the website using a keyboard or speech recognition software
- listen to most of the website using a screen reader (including the most recent versions of JAWS, NVDA and VoiceOver)
We aim to make the CQRS National Web Application simple to use and understand for all users, including healthcare professionals and commissioners.

Compliance statusThis web application is partially compliant with the Web Content Accessibility Guidelines version 2.2 AA standard due to the non-compliances and exemptions listed below.
Non-accessible contentThe following items are currently non-compliant with the Web Content Accessibility Guidelines version 2.2 AA, although we are actively working to address these issues as part of an ongoing programme to improve the accessibility of this web application.
Issues with links (WCAG 2.4.4 Link Purpose)- Link names are not clear - affected links with link names are not clearly defined
- Two or more links with same names and different destinations
- Link names with full stop
- Redundant links - download component has redundant links
- Button names are not clear
- Alt text does not match with the image - alt text does not match with the image in some cases
- Alt text is not read by the screen reader - alt text is available but is not captured by the screen reader in some cases
- Image is not captured by the screen reader - reads out only the title and skips the image and its content
- Alt text is not read by the screen reader clearly
WCAG 1.3.1 Info and Relationships
- Headings are not captured - some headings are not captured in the webpage by screen readers or are missing
- Heading structure is not proper - some headings do not follow the correct structure
- Tables do not have headers
- A group of check boxes or radio buttons is not enclosed in a fieldset - a group of check boxes or radio buttons is not enclosed in a fieldset
- Heading is not displayed fully in mobile view
- Title is not displayed fully in mobile view
- The download button overlaps the logo in mobile view
- Lost content - page title is not fully displayed in mobile view
- Headings display affected when the page is zoomed in to 300% and above
- Lost content at 250+% zoom - the main heading of the page is not fully displayed when the page is zoomed to 250 + zoom level
- Lost content at 400% - page title is not fully displayed at 400% zoom level
- Low contrast - Code block - content inside the code block shows low contrast error
WCAG 2.4.6 Headings and Labels
- Empty headings
- Issues with navigation
- Menu tabs - aria-labelled by to name regions and landmarks
- Some pages do not use aria-labels to provide labels for objects
- Focus order is illogical - focus order not in the correct order
- Some focus indicators either cannot be visually located, or are highly visible
- Some content cannot be captured by screen readers
- Labels that include text or images of text
- Label in name changes presentation of user interface
- Content not captured by the screen reader
WCAG 3.3.2 Labels or Instructions
- Form label is not associated with a form control
- Frame does not have a title
Some archived content and legacy documents were created before 23 September 2018 and may not be fully accessible. We do not plan to update these as they are no longer actively used.
What we are doing to improve accessibilityWe are currently in the planning phase of a major project to improve accessibility to ensure compliance with the WCAG 2.2 AA standard.
Preparation of this accessibility statement
This statement was prepared on 20 October 2025. It was last reviewed on 20 October 2025.
The CQRS National web application underwent a full review by AGEM on January 2025, following a self-assessment and automated testing using Axe, WAVE, and Lighthouse tools.
